Q: Does long foreskin / long prepuce / phimosis affect sex life?
A: For patients with long foreskin/ phimosis, the prepuce would cover the urethral opening even after erection. This would certainly affect ejaculation and sexual life.
There is often urine, debris and bacteria trapping between the glans of penis and the foreskin, causing unpleasant odor. Besides, this would increase the chance of bacterial balanitis, posthitis, urinary tract infection etc. Through sexual intercourse, female partners might have increased chance of vaginal infections, pelvic inflammatory disease or urinary tract infections etc. Long foreskin / long prepuce / phimosis are also associated with easy slipping out of condom. During ejaculation, the semen is trapped between the foreskin and the glans of penis, and this affects fertility. As the glans of penis is fully covered by the foreskin in most of the time, it may increase the sensitivity of the glans of penis, once it is exposed, resulting in premature ejaculation.

Q: Does phimosis / long foreskin / long prepuce/ balanitis/ posthitis affect the growth of the penis?
A: Patients with long foreskin / long prepuce and phimosis usually do not retract the foreskin during urination. There is often urine, debris and bacteria trapping between the glans of penis and the foreskin, causing unpleasant odor. This would increase the chance of bacterial balanitis, posthitis, urinary tract infection etc.
After balanitis or posthitis, the surrounding tissue may form fibrous / scar tissue. This wound in turn causing the prepuce constricting the glans of penis tightly and affecting urination, penile growth or resulting in penile distortion.

Q: Do I need to clean my foreskin / prepuce?
A: Yes. For patients having long foreskin / long prepuce / phimosis, there is often urine, debris and bacteria trapping between the glans of penis and the foreskin, causing unpleasant odor. Besides, this would increase the chance of bacterial balanitis, posthitis, urinary tract infection etc. You need to clean the prepuce and glans of penis after urination.
Q: How do I clean the foreskin?
A: The adult male should retract the foreskin and clean the foreskin and the glans with water(+/- soap / bath gel) every day. He should also clean the prepuce and glans of penis before and after sexual intercourse.
Q: Do all men need circumcision?
A: No. If you are suffering from phimosis, long foreskin (long prepuce), paraphimosis or foreskin diseases (such as flat warts, genital warts, sebaceous cysts, recurrent balanitis, recurrent posthitis, recurrent urinary tract infection, seborrheic keratosis, balanitis xerotica obliterans, and prepuce skin cancer), then you need to consider circumcision.
Q: How many days of sick leave do I need after circumcision?
A: Traditional circumcision is a surgical procedure involving cutting of the foreskin with scalpel or scissor and closing of the wound with ordinary surgical sutures. There is usually more blood loss during the surgery and the post-operative pain is usually moderate to severe. Normal saline, distilled water, or antiseptic solution is required for post operative wound cleansing every time after urination. Students are expected to be absent from school for about 1 - 2 weeks. Adults are expected to be absent from work for 1 - 2 weeks.
After new generation laser circumcision and adult ring circumcision, patients can return to office work immediately. These surgeries has an advantage of less blood loss during surgery, less wound discharge post operatively, not requiring regular wound cleansing after urination, less post operative pain sensation, better looking wound etc.

Q: Will circumcision improve my sexual function?
A: It may. The glans of penis of patients, with phimosis and long prepuce / long foreskin, are usually more sensitive once the prepuce is retracted and the glans of penis is exposed. This hypersensitivity of the glans of penis may result in premature ejaculation. After circumcision, the skin of the glans of penis will become dry and will gradually thicken, the hypersensitivity of the glans usually fade away in few weeks time. This may improve premature ejaculation and thus sex function.
Q: When can I resume sexual activity after circumcision?
A: Patients normally can resume sexual activity 4 - 6 weeks after circumcision.
