Frenulum breve

The frenulum of the prepuce is an elastic band and its main function is to fix the prepuce to the inferior part of the glans of penis.  If there is frenulum breve, simple erection may cause pain at the frenulum.  Sometimes, during vigorous sexual intercourse, the frenulum may break and bleed.

Bacterial infection and scarring may happen after a tear of the frenulum.  Scarring results in further shortening of the frenulum and there may be repeated tearing after subsequent sexual intercourse.  

• If there is frenulum tear and non stop bleeding, you should find an experienced doctor for urgent surgical intervention.

Treatment
Frenuloplastyis quick and simple surgical treatment for frenulum breve or frenulum tear.  First of all, the frenulum is divided and extended by scissors.  Then the new frenulum is constructed with sutures. Frenuloplasty is a short surgery with minimal blood loss and postoperative pain.  The postoperative care is simple.  All you need to do is to keep the wound dry.   You can return to work immediately after surgery.  Frenuloplasty can be done with circumcision at the same time. Water soluble sutures will dissolve in few weeks time. There is usually no need for taking off the sutures.

Precautions for adult frenuloplasty

For frenuloplasty performing in the hospital:

Before surgery:

• Inform doctor for special medical condition(s) and current drug(s) use
• Shave the pubic hair and have a shower in the morning before surgery to reduce the bacterial load around the wound
• Keep nil by mouth six hours before operation
• Void immediately before surgery and relax

After surgery:

• Take a rest and learn the steps for wound care
• Expect to leave on the same day or the day after
• Wear tight underwear (for reduction of penile movement) and loose pants
• Follow up in the clinic 3-7 days after operation
• Keep wound dry all the time

For frenuloplasty performing in the clinic:

Before surgery:

• Inform doctor for special medical condition(s) and current drug(s) use
• Shave the pubic hair and have a shower in the morning before surgery to reduce the bacterial load around the wound
• Have a light breakfast before operation
• Void immediately before surgery and relax

After surgery:

• Take a rest of about 15-30 minutes
• Expect to leave after learning the steps of wound care
• Wear tight underwear (for reduction of penile movement) and loose pants
• Follow up in the clinic 3-7 days after operation
• Keep wound dry all the time
